Functional capacity
Assessments relating to how you undertake one or more of the following activities:
Communication: includes being understood in spoken, written or sign language, understanding others and expressing needs and wants by gesture, speech or context appropriate to age.
Social interaction: includes making and keeping friends, interacting with the community, behaving within limits accepted by others, coping with feelings and emotions in a social context.
Learning: includes understanding and remembering information, learning new things, practicing and using new skills.
Mobility: this means the ability of a person to move around the home (crawling/walking) to undertake ordinary activities of daily living, getting in and out of bed or a chair, leaving the home, moving about in the community and performing other tasks requiring the use of limbs.
Self-care: means activities related to personal case, hygiene, grooming and feeding oneself, including showering, bathing, dressing, eating, toileting, grooming, caring for own health care needs.
Self-management: means the cognitive capacity to organise one's life, to plan and make decisions, and to take responsibility for oneself, including completing daily tasks, making decisions, problem solving and managing finances.
Assistive technology (AT)
Assessments relating to aids and equipment required at home, in the community and the workplace:
AT may be equipment or systems which help you with moving around your home or community, communicating with other people, processing information, and other daily tasks.
Supports may be included in your NDIS plan where it is a reasonable and necessary support that will meet your needs and help you pursue your goals.
We are able to assess your needs and situation, and identify the most appropriate AT. Depending on the risks associated with your disability or the environment where you need to use it, an item of AT may increase in complexity. For more complex AT requests you may be required to submit a report from us along with associated quote(s).

Minor & Complex home modifications (HM)
Assessments relating to minor or complex modifications to your primary residence:
Home modifications are changes to the structure, layout or fittings of your home so you can safely access it and move around comfortably.
The NDIA needs sufficient evidence to decide which HM supports are reasonable and necessary to include in a participant’s plan.
We are able to assess your needs and situation, and identify the most appropriate HM.
As with any building work, there are laws and regulations that need to be adhered to when undertaking complex home modifications under the Scheme. Further detail is available in the Operational Guidelines.

Home visits
How do home visits work?
Face-to-face home visits are performed in the comfort of your own home and are the preferred option for all initial assessments. You will be contacted directly to arrange a suitable time and can expect the appointment duration to be between one to two hours in length.
ONLINE CONSULTATIONS
How do online consultations work?
Online consultations are conducted using Telehealth, a software which is designed for the Australian healthcare environment, and as such meet Australian information privacy and security regulations Telehealth consultations are conducted via a secure video platform to provide assessment, education, support and recommendations. The video platform used is compliant with Australian healthcare regulations. If you are seeking assessment and recommendations regarding your physical home set-up, it is recommended that you use a portable device such as a tablet or mobile phone (rather than a desktop computer) to enable you to virtually walk through your home environment.
What technology do I need to use Telehealth?
Your smartphone, tablet or a computer with the latest version of Firefox, Safari, Chrome or Microsoft Edge browsers are compatible with the platform used for consultations. You will be emailed with further instructions prior to your call and there is no need to download anything, you will have access via a simple link.
